You also need to create and upload a gif image to be used as a go button. Change the image path and image name with your image name and url. Also change the option value URL's with your own URL's. It does not use the windows default html go button or any other gif / jpg, "Go" image. In this script you just select the option and the form is automatically submitted there, immediately. This is most simple option of sending form. Also change the option value URL's with your own URL's. You can add more URL's to the form just by copy-paste the option values. But remember it does not give the user a chance to think if he wants to go to the new location or not?.
